In Case of Emergency (ICE) Here’s how it works: • If you are under 18, make sure your ICE contact is a parent or guardian authorized to make decisions on your behalf. • Make sure the person whose name and number you are giving has agreed to be your ICE contact. • Your ICE contact should have a list of people to call on your behalf (next of kin, your place of work, etc.). • Provide an ICE contact’s number that is easy to reach. • Alert your ICE contact to any medical conditions that could affect your emergency treatment (your allergies, current medication, etc.). For ICE-related information at a J&W campus, contact the following: Providence, RI Captain James Beauvais (401-598-2946) or Lt. Harvey DuPerry (401-598-2955) North Miami, FL Yakir Fernandez (305-892-7603) Charlotte, NC Director Robert Allen (980-598-1905) Denver, CO Director Michael Eaton (303-264-3115) or Project Manager Kayce O’Brien (303-256-9377) © 2000 Johnson & Wales University All rights reserved.
