Please review the List of I-9 Acceptable Documents (88K PDF). Please note the following documents have been removed from the list of acceptable identity and work authorization documents:
• Certificate of U.S. Citizenship (List A #2)
• Certificate of Naturalization (List A #3)
• Unexpired Reentry Permit (List A #8)
• Unexpired Refugee Travel Document: Form I-151 (List A #9)

In accordance with United States Citizen and Immigration Services requirements, all University employees must complete the Federal I-9 Form which verifies the identity of the employee and their eligibility to work in the United States. This form requires employees to produce certain original documents for review by a designated University representative. Photocopies or faxes of the document cannot be accepted.

Community Service Work-Study
• Local venues in community service agencies in and around the Johnson & Wales University Campus.
• Premium wage of $8.25 per hour
• Flexible work schedules including some weekend or night hours
• Opportunities to make a difference in your community by serving the needs of children, the elderly, the homeless and other worthy causes

To apply, complete the form and return it to Student Employment by August 1, 2007

Direct Deposit (108K PDF) A checking or statement savings is necessary in order to complete the direct deposit form. Submit the completed form to your campus. If utilizing a checking account, a voided check will need to be submitted with the completed form.

Stop Direct Deposit (72K PDF) Complete this form to stop a direct deposit. The completed form should be submitted to your campus.

Replacement Check Request (72K PDF) Complete this form if you have lost your paycheck and are seeking a replacement check.
